You can fill SCRA application form online only.The dates are
SCRA 2013
date of notification: 13-10-12
last date for form filling: 12-11-12
date of exam: 20-1-2013(sunday)
You can read all the requirements for the exam from the pdf attached below.

For fees payment:
When you fill the online application form it generates a receipt in which the fees is mentioned.
You have take the receipt to nearest bank for example SBI there you have to pay the fees.
They will provide you a code which you have to enter in the space mentioned in online application form.
After you enter the code your form will be submitted and printout will be generated of copy of application form.
