The eligibility criteria for NDA/NA exam is:
1.You must be an unmarried citizen of India.
2.Your age must lie inbetween 16 to 19.5 years on 1st august of the year of exam.No age relaxation to any category candidates.
3.You must be 12th pass/appearing student with science stream and physics and maths as one of your subjects.

Since you are 12th appearing student but have not mentioned your stream we take you as science stream student and hence you are eligible to apply for NDA.

NDA/NA exam is conducted by UPSC two times in a year. The exam is conducted for the posts in defence like Air Force,Navy,Army.
There is online mode to apply for UPSC NDA/NA Exam. You have to apply for the exam online. And yes 12th appearing candidates can also apply for the exam.

Eligibility Criteria For NDA/NA:

For Army wing:
