Hi Aspirant,
Yes friend there is separate medical test in UPSC exams.
After qualifying all the stages of exam procedure final stage is medical.
You will be called for medical test after qualifying written exam or interview.
If you qualify the medical test your name will be include in final merit list.
UPSC allows two mode to fill up the application form

You can purchase the application form from head post office.
Fill the application form carefully, paste your photo and stick CRF stamp of requisite application fee and send to the prescribed address.
You will get an email as a confirmation.
Online mode
You can apply for UPSC exam through Online
Open http://upsconlinen.nic.in
Choose the exam name.
Here you will have to complete application form in two parts
In first part you will fill the basic details like name, father name, qualification etc. and submit the form.
In second form you will have to fill payment details, upload your photo and signature and submit the form.
Payment mode: SBI Challan, Net banking or Debit card.

Yes, UPSC has a separate medical and physical standards examination. THis test is conducted after the personality test is over by a special medical board consisting of experts from different medical fields. THe following medical and physical criteria will be checked
1) height & weight
2) general Eyesight
3) vision acuity - one for distant vision and another for near vision
4) colour perception , night vision
5) blood pressure
6) Ear, nose and throat checking
7) Radiographic examination of the chest for detecting abnormality in heart or lungs, which cannot be known by direct physical examination
The candidate will be subjected to the above mentioned tests.
The form will be given only when you are taking the medical test.
