friend you should not to fill any special form as belonging to the medical field.
you have to fill simple form same as all the other candidates are filling the form,
if you don't. know how to fill the application form than you have to follow the instructions given below.
1. at first you should go on the website of upsc http://www.upsconline.nic.in
2. click on ' online application form for various upsc examination.'
3. select the examination and click on 'part -1 registration'
4. fill the general details about yourself in the part -1 and click submit.
5. take a printout of chalan copy.and go to the bank.
6. give him/her(BANKER) chalan copy with money written on it .and she will give you a
transaction id on the copy of chalan.
7. now come again to your computer and open the 'part - 2 registration.'
8. provide the transaction ID and registration ID to open the part-2 registration.
9. now provide your scanned photograph,signature and left hand thumb impression(if required)
and click on submit.
10. after submitting the application form you will get a conformation e-mail in the mailbox.take a print out of your application form ,it will help you for further correspondence to upsc.

Hi,As far as fill any special form along with UPSC application for CSE if you belong to medical field is concern no you don't fill any special form, you should fill the same form as by other candidates,On the other hand procedure for online registration is concern for that, first of all you should go to the upsc official website.(www.upsc.gov.in )now you will find a link online application for various exams click on that and select thepart-1 registration for civil services exam.now you should give all your details as required.Good luck.....

you do not need to fill any special form along with CSAT form. you just need to fill CSAT form as other candidates do.
INSTRUCTIONS TO THE CANDIDATES FOR FILLING ONLINE APPLICATIONS for CSAT Exam:

- Candidates are required to apply Online using the Website http://www.upsconline.nic.in
- Select the exam in which you want to apply.
- Candidates will be required to complete the Online application form containing two stages viz. Part-I and Part-II as per the instructions available in the above mentioned site through drop down menus.
- The candidates are required to pay a fee of Rs. 50/- (Rupees Fifty Only)(Rs. 100/- in case of offline filling)[excepting SC/ST/Female/PH candidates who are exempted from payment of fee] either by remitting the money in any branch of SBI by cash, or by using net banking facility of State Bank of India/State Bank of Bikaner & Jaipur/State Bank of Hyderabad/ State Bank of Mysore/State Bank of Patiala/State Bank of Travancore or by using any Visa/Master Credit/Debit Card.
- Before start filling up Online Application, a candidate must have his photograph and signature duly scanned in the .jpg format in such a manner that each file should not exceed 40 KB each and must not be less than 3 KB in size for the photograph and 1 KB for the signature.
- Applicants should avoid submitting multiple applications. However, if due to any unavoidable circumstances any applicant submits multiple applications then he must ensure that the applications with higher RID is complete in all respects.In case of multiple applications, the applications with higher RID shall be entertained by the Commission and fee paid against one RID shall not be adjusted against any other RID.
- The applicants must ensure that while filling their application form, they are providing their valid and active e-mail Ids as the Commission may use electronic mode of communication while contacting them.
- The applicant must apply for the examination well in advance without waiting for the last date.

No there is no compulsion to fill any other application form with the form or preparation of civil service exam.
in my suggestion you should only do the preparation of civil service exam in one time.
because it is quite tough exam and not so easy to crack.

Here you will compete application process in two parts in first part you will fill all the basic details and submit the application form.
after submission form download the challan paper and submit it in SBI branch with fee of Rs. 100.
Now complete the second part. In second part you will have to fill the transaction ID of challan, Upload your photo and signature copy.
Now registration process completed successfully

No you don't need to fill any special form. You have to apply for Civil Services Examination.
Civil Services Examination is conducted by UPSC every year for the recruitment of candidates for the post of IAS, IRS, IPS.
Exam Format of Civil Services Examination
The Civil Services Exam is a paper based test and consists of two successive stages:The basic eligibility criteria for Civil Services Exam are as follows:
- Preliminary Examination � comprising 2 exams � General Studies and CSAT
- Civil Services Main Examination (Written and Interview)
