No you dont need to do Post graduation.
And you can not do PG once you joined UPSC as a civil servant.
you can go with Combined medical services exam, Under UPSC only. exam notification comes in month of november to december.
you have optional subject as medical sciences as well, so its plus point for you.

Exam takes place in month on may to august every year.

Yes,you can definitely go for the Combined Medical services examinations after the Bachelor's degree in medicine and surgery .This exam is conducted once every year by the UPSC.
The eligibility criteria for the CMS exam is as below:
>>Candidates who has passed the written and practical parts of the M.B.B.S are eligible for the exam .Final year MBBS candidates can also apply for the exam.
Age limits : Below 32 years on the 1st January of the examination.
Nationality:
>>You must be an Indian citizen.
Examination pattern:
4.Important dates for the CMS 2013 are:
Date of notification:06/04/2013.
Last date of receiving application:06/05/2013
Date of examination:30/06/2013.
