dear friend,
being a Engineer Medical Science is not helpful for you in preparation. as it is not related to your field.
i advice you to select your core subjects like Physics and Mathematics as your optional subjects. because being a Engineer you have knowledge of these subjects which will be helpful for you in preparation and cracking the exam.

Medical science subject will be very vast and extensive. Only candidates with MBBS or similar qualification only can take it as an optional. Since you are an Engineering graduate it will be very difficult for you to cover the entire subject. Have you seen the syllabus for medical science. It is not easy for an ENgineering graduate to study medical science subject all of a sudden after 4 years of Engineering studies where you would not have read or learnt anything related to medical science. First look at the syllabus, its not like higher secondary zoology syllabus. The syllabus is of a medical professional degree standard. Thus it will be completely alien to you, you cannot master the subject just for upsc mains exam. So think carefully and decide. Look at other optional subjects offered by UPSC and then decide. Even if you have interest in medical subject, it is not a good decision to take it as an optional since you dont have any prior exposure on this subject

I think you are making a fun of Civil Service exam.
But I don�t know why you want to do so. And you why you want to destroy your career before starting.
Friend what do you think about Civil exam.
Is it very easy and you can crack it smoothly.
If you think as I mentioned then you are fully wrong.
My dear friend civil service exam is the toughest exam of India and 2nd toughest exam of the world.
There are so many candidates who are struggling to crack Civil exam from several years.
Very few candidates crack the civil exam in first attempt.
So friend never think to choose medical science as optional subject. it is just opposite of your stream.
You are engineer why don�t you choose the subject which is concerned to engineering and related to engineering background and about that subject you are confident.
You can choose the mechanical, Civil, physics , chemistry, math as optional subject which belong to your stream and you are familiar with them.
You can choose also other subject in the list of optional subject.
